Registered Nurse (RN) – Regional Home Health Visit Nurse
About The Position
This role is designed for nurses who like experiencing different communities, supporting multiple clinical teams, and caring for a diverse patient population while maintaining the independence that makes home health nursing so rewarding. Instead of being assigned to one permanent territory, you'll help wherever patient demand is highest, giving you the opportunity to broaden your clinical experience while making an immediate impact across our organization. No two days are exactly alike. One week you may support patients in the north metro. The next, you may spend time helping a team in another community. That flexibility keeps your work interesting while allowing you to become familiar with different clinicians, referral sources, and patient populations throughout our service area. For nurses who enjoy learning, adapting, and staying engaged, it's one of the most rewarding positions in home health.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, or skilled nursing preferred
- Strong assessment and critical thinking skills
- Comfortable working independently in a field-based environment
- Flexible and adaptable to changing daily assignments
- Reliable transportation and willingness to travel throughout the Twin Cities Metro
Responsibilities
- Provide skilled RN home health visits throughout the Twin Cities service area
- Complete comprehensive nursing assessments
- Perform wound care, medication management, injections, and other skilled nursing services
- Educate patients and caregivers on disease management and recovery
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Support multiple territories based on patient needs and census
- Complete accurate documentation using Homecare Homebase (HCHB)
- Help ensure patients receive timely, uninterrupted care
