About The Position

Suncrest Hospice aims to elevate the standard of hospice care by delivering exceptional service. This is achieved by ensuring adequate staff-to-patient ratios, which allows for increased clinical visits and reduced clinician caseloads. Suncrest Hospice is a CHAP-certified organization. We are seeking individuals dedicated to providing the highest quality of care to patients and their families.

Requirements

Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
Ability to collaborate effectively within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
Ability to work independently on a day-to-day basis, managing activities such as phone, email, and written communication, and meetings with healthcare professionals.
Understanding of hospice rules and regulations.
Time management skills to manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
Ability to learn an electronic medical records system.
Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Nice To Haves

Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Work closely with patients on a 1-1 level but get strong support from a patient care team.

Responsibilities

Provide care and services for a group of patients.
Visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities.
Serve as a contributing member of the interdisciplinary team.
Provide expertise in psych-social assessments, interventions, and knowledge of community resources.
Work in a Case Management, Admissions, or Resource role.
