About The Position
A Registered Nurse (RN) is needed for a PRN (as-needed) Home Care position in Cleveland, OH. Join a growing Home Care Company with flexible scheduling and competitive per-visit pay. This is a PRN position covering SOC (Start of Care), ROC (Resumption of Care), Recerts, and Discharges. Pay is $50-$60 per visit (negotiable).
Requirements
- Current state license to practice as a Registered Nurse in Ohio.
- Graduate of an accredited nursing program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong assessment, communication, and time-management skills.
Nice To Haves
- OASIS and home care experience preferred.
Responsibilities
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
- Perform comprehensive patient assessments and develop individualized care plans (OASIS).
- Coordinate care with physicians, therapists, and other members of the care team.
- Provide skilled nursing services and patient/family education.
- Monitor patient progress and adjust the plan of care as needed.
- Complete accurate, timely clinical documentation.
