The mission of this role is to deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Requirements

Active Michigan RN license
Reliable transportation
Valid driver's license
Auto insurance

Nice To Haves

Home health experience strongly preferred
Home health / OASIS experience strongly preferred

Responsibilities

Deliver excellent home health care to patients in Detroit.
Ensure visit quality with OASIS accuracy.
Drive low avoidable-hospitalization rates and strong patient-experience scores.
Participate in case conferences/team huddles to improve quality.
Be responsive and excellent to referral sources.
Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
Provide direct, high-acuity patient care, with a focus on wound care.
Build and manage the plan of care with physicians.
Supervise LPNs and HHAs on your cases.
Be the face of Adaptive to patients, families, and referral sources.
