Requirements
- Valid, unencumbered Registered Nurse license in Pennsylvania.
- Minimum of 1-2 years clinical nursing experience; prior home health or community health experience preferred.
- Strong clinical skills: wounds, IVs, medication management; ability to assess, triage, and identify potential complications.
- Excellent communication skills—able to interact well with patients, families, and multidisciplinary team.
- Strong organizational skills; ability to manage a caseload, prioritize visits, travel between home sites.
- Self-motivated, independent worker with good problem-solving ability.
- Reliable transportation, valid driver’s license, and willingness to travel within assigned counties.
- Up-to-date certifications: CPR, possibly first aid; any other required state/home health nursing credentials.
- Ability to pass background check and required health screenings/immunizations.

- Prior home health or community health experience preferred.
Responsibilities
- Conduct comprehensive nursing assessments on admission and at regular intervals: physical, psychosocial, environmental.
- Develop, implement, and update individualized care plans in collaboration with physicians, therapists, aides, social workers, and family.
- Administer medications, treatments, and therapies as ordered (e.g., wound care, IV, injections) and document fully.
- Monitor patient vital signs, condition changes, and response to treatment. Report findings and adjust care plan as needed.
- Provide patient/family education: disease processes, medication management, wound care, safety in the home, etc.
- Coordinate care: collaborate with therapists (PT, OT), social work, home health aides, and other providers to ensure an integrated approach.
- Supervise, mentor, and support Home Health Aides and LPNs under the RN’s purview (as applicable).
- Ensure all documentation is accurate, timely, in compliance with agency policy, payer requirements (Medicare/Medicaid), and regulatory standards.
- Conduct regular home environment safety evaluations (infection control, fall risks, cleanliness, proper equipment usage).
- Participate in quality assurance, audits, performance improvement activities.
- Track outcomes, incidents, and client satisfaction.
- Ensure compliance with all legal, regulatory, and ethical standards (e.g. patient confidentiality, state licensing, mandatory training).
