About The Position

This position seeks a compassionate and experienced Registered Nurse (RN) to provide in-home care for clients in private duty home health services. The role involves assessing patient health needs, evaluating caregiver skills, performing supervisory visits, and collaborating with families and caregivers to promote safety, comfort, and overall well-being. The ideal candidate possesses strong clinical judgment, excellent communication skills, and a passion for delivering high-quality, personalized care in the home setting.

Requirements

  • Active RN license in the state of practice.
  • Strong clinical assessment and patient care skills.
  • Excellent communication and interpersonal skills to work effectively with patients, families, and caregivers.
  • Ability to work independently and manage a flexible schedule.
  • Reliable transportation and a valid driver's license.
  • Proficiency with electronic medical records (EMR) and basic computer skills.
  • CPR and other relevant certifications as required.
  • Willingness to travel up to 75% of the time.

Nice To Haves

  • Previous experience in home health, private duty, or outpatient nursing.

Responsibilities

  • Assess patients' health status and needs in the home setting.
  • Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
  • Educate patients and families on disease management, health maintenance, and safety.
  • Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
  • Maintain accurate and timely documentation of care in compliance with company policies and regulations.
  • Promote a safe, comfortable, and supportive environment for patients in their homes.
  • Conduct caregiver evaluations and assess caregivers' skills to ensure quality of care.
  • Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.
