Requirements

  • A heart for hospice care and a commitment to exceptional service
  • A valid Registered Nurse (RN) license in good standing in the state of Hawaii
  • Current CPR certification
  • A valid driver's license and reliable transportation
  • Strong observational, documentation, and communication skills
  • A flexible, collaborative, and patient-focused approach to care

Responsibilities

  • Develop and implement individualized care plans based on current and prior medical history
  • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
  • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
  • Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
  • Administer physician-ordered medications and treatments with skill and empathy
  • Educate patients and their families on care techniques and symptom management
  • Track and communicate changes in patient condition, ensuring timely and effective responses
  • Maintain detailed and accurate clinical documentation reflecting patient progress and quality of care
  • Collaborate with interdisciplinary team members to ensure comprehensive care delivery
  • Participate in on-call duties, as defined by the on-call policy
  • Perform additional duties as assigned in support of patient care and team success
