About The Position

VON Canada is a not-for-profit organization providing home and community care services for over 125 years. Focusing on person-centred care, we provide the support people need and want, at home and in the community, so they can live healthier, happier lives surrounded by loved ones. This role involves delivering holistic, person-centred care to clients in their homes, clinics, virtually, and through community outreach. Visiting nurses provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, and counselling to individuals and families.

Requirements

• Must be licensed as a Registered Nurse in the province of practice
• Fundamentals of Palliative Care, or an equivalent required
• CAPCE or equivalent required or agreement to complete upon hire
• Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
• A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
• Must be able to wear Personal Protective Equipment (PPE)

Nice To Haves

• Community health experience is an asset
• Three (3) years of experience in the community or acute care setting, with a focus on palliative care is preferred

Responsibilities

• Client assessments to develop individual care plans
• Provide primary care in collaboration with other healthcare professionals
• Simple and advanced wound care, including complex and negative pressure wounds
• Chronic Disease Management, e.g. diabetes care and education
• IV therapy and CVAD management
• Phlebotomy and specimen collection
• Dialysis Management
• Rehabilitative programs
• Palliative and End of Life Care
• Medication management
• Other critical nursing skills
