About The Position

Mercy Health is seeking a Registered Nurse (RN) for their Outpatient Infusion Center at Fairfield Hospital. This position involves delivering patient care using the nursing process, including data collection, assessment, problem identification, care planning, and evaluation. The role also includes coordinating patient teaching and collaborating with health team members. The nurse will conduct initial and ongoing patient assessments, analyze data, create care plans, administer medications safely, and serve as a point of contact for care coordination. A key aspect of the role is acting as a patient safety advocate through participation in quality improvement initiatives.

Responsibilities

  • Delivers patient care utilizing the nursing process through data collection and assessment.
  • Identifies and determines the priority of patient’s problems/needs.
  • Evaluates the process and outcome of nursing care.
  • Coordinates, provides and directs patient teaching activities.
  • Coordinates the care provided by health team members.
  • Conducts initial and ongoing patient assessment in collaboration with the interdisciplinary care team.
  • Analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness.
  • Administers medications in a safe manner consistent with the State of Practice and organization policies and procedures.
  • Serves as point of contact for patients' care coordination throughout hospital departments.
  • Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
  • Incorporates the professional practice model into care delivery.
