About The Position

The Saint Francis Hospital Outpatient Heart Failure Program provides care to patients from surrounding communities experiencing various stages and types of heart failure. The program has consistently achieved Gold Plus status in Heart Failure from the American Heart Association and Advanced Certification in Heart Failure from The Joint Commission, recognizing high standards for treatment. The Outpatient clinic within Saint Francis Hospital specializes in different degrees of heart failure, focusing on improving patients’ quality of life and optimizing their disease process through guideline-directed medical therapy (GDMT) and evidence-based care. Clinic leadership supports new hires during orientation, tailoring the process to individual needs.

Requirements

  • Graduate of an accredited nursing program.
  • Current Licensure as a Registered Nurse RN in Connecticut.
  • BLS (AHA/ARC) certification.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) degree.
  • 2-3+ years of Cardiology experience.
  • ACLS certification.
  • Stroke Certification.

Responsibilities

  • Provides heart failure disease management education.
  • Assists in managing patients within the Outpatient Heart Failure Program.
  • Works closely with advanced heart failure MD and advanced practice providers in managing patient medical issues.
  • Provides patients with resources for medication therapy and care.
  • May provide infusion therapy for patients requiring diuresis in the clinic.
  • Provides patients with appointments in clinic in collaboration with APPs for medication guidance and disease management.
  • Assists with in-basket inquiries within the electronic health record.
  • Makes necessary phone calls to patients and other providers to assist in managing patient care.
  • Provides inpatients that are late day discharges with scheduled appointments and bedside education prior to their discharge.
  • Collaborates as part of a strong interdisciplinary team including MDs, APPs, RNs, and MAs.
