About The Position

Registered Nurse (RN) or Licensed Practical Nurse (LPN): officially we title this job an RN or LPN, but in other centers it may be called a Floor Nurse or Staff Nurse. Transitional Care Management has 12 different locations throughout Chicagoland. Multiple centers give you the opportunity for advancement. Bourbonnais Terrace is a Specialized Mental Health Rehabilitation Facility (SMHRF) that has 197 beds and provides rehabilitative treatment to individuals seeking to recover from their illness. The Administrator often says, “Our focus is recovery and rehabilitation”. With many restaurants located here, we are just 2 minutes west from I-57. We focus on building relationships with our residents and families. In time, you may feel like they are your family too. Our Activities/Events department goes beyond bingo. We focus on the individual patient and as a result they are more engaged with you. This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers. Each center is unique; offering you a variety of long-term career opportunities.

Requirements

RN or LPN license in IL
Prior experience in a SNF or ICF preferred
Prior experience or desire to work with behavioral population
Electronic medical records experience; specifically Point Click Care (or PCC) preferred
New grads are welcome
