Registered Nurse (RN) or Licensed Practical Nurse (LPN) – Full-Time | Home Care
About The Position
Abernathy Home Care is seeking an experienced RN or LPN for a full-time, field-based role supporting ASAP (Aging Services Access Point) community-based clients. This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management. You will be working independently in the field while collaborating closely with case managers, families, and the internal team. The role focuses on ASAP consumer care, requiring strong knowledge of community-based services, compliance, and supervisory oversight. This is not a hospital or facility-based position, nor is it remote or a non-clinical/companion role.
Requirements
- Active RN or LPN license in Massachusetts
- Minimum 1 year of home care experience preferred, but not required (new grads encouraged to apply)
- ASAP experience strongly preferred
- Must be comfortable working independently in the field
- Reliable transportation and willingness to travel daily
- Strong assessment and documentation skills
Nice To Haves
- Bilingual Spanish preferred, but not required
Responsibilities
- Complete in-home supervisory visits for HHA/PCA caregivers
- Conduct client assessments and reassessments
- Ensure compliance with ASAP program requirements
- Communicate with ASAP case managers and families
- Monitor client conditions and report changes
- Maintain accurate and timely documentation
Benefits
- Salary: $90,000 – $105,000 annually (based on experience)
- Mileage reimbursement provided
- Full-time, stable Monday–Friday schedule
