About The Position

Lehigh Valley Health Network (LVHN) is seeking an On-Call Registered Nurse (RN) for Hospice Weekend Nights. This role provides clinical support and assists in the coordination and delivery of patient care services during or after normal business hours. The nurse will perform data/order entry, clinical record reviews, and other clinical tasks related to patient care. They will plan, organize, and direct care in collaboration with the interdisciplinary team to meet the needs of the patient and family in their homes and communities. Requirements

Specialized Diploma from an NLN approved/accredited nursing program
1 year recent experience providing direct care in a healthcare setting

CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years

Responsibilities

Performs telephone triage for patients calling during or after hours utilizing critical thinking skills and sound clinical judgment.
Performs assessments of patient status, provides direct nursing care, and addresses immediate needs related to physical, psychosocial, and environmental factors.
Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, pain management, symptom control, home care, hospice, and discharge planning.
Performs data/order entry, clinical record reviews and other clinical tasks related to the provision of patient care.
Provides hand-off communication to RN covering next shift, interdisciplinary team members, and/or Clinical Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.
