About The Position

Located adjacent to Glens Falls Hospital, our Surgical Unit offers consult on general surgery, thoracic surgery, urology, and orthopedic surgery (total joint and Mako Smartrobotics). This office features a modern, state-of-the-art, specialty physician practice space for patient consultations (ages 18+). As a Orthopedic Nurse Navigator you will serve as a single point of contact to facilitate transition throughout the continuum for patients undergoing total joint surgery, from decision throughout the post-discharge phase of care. You will work hand in hand with the Nurse Managers in Post-Anesthesia, Day Surgery, an AM Admissions.

Requirements

  • Baccalaureate degree in Nursing required
  • Minimum 2 years Medical/Surgical nursing experience, with orthopedic surgery experience required
  • Advanced clinical decision-making skills; documented experience providing patient education and collaborating with other disciplines
  • Demonstrated leadership skills in professional nursing practice.
  • Advanced assessment and clinical decision-making skills
  • Strong collaboration and communication skills; able to articulate and adapt communication to varied educational levels and unique patient needs
  • Flexibility, creativity, leadership and organizational skills are essential
  • Ability to demonstrate sensitivity to individuals and families diverse care needs
  • Ability to collaborate with physicians and various other levels of staff
  • Excellent verbal and written communication skills
  • Strong computer proficiency required, including Microsoft Office products, electronic medical records, databases and Internet
  • Current license and registration to practice Registered Professional Nursing in New York State

Nice To Haves

  • Prior experience in Care Coordination or Nurse Navigator role is preferred
  • Specialty nursing certification preferred

Responsibilities

  • Coordinates comprehensive care for patients undergoing total joint surgery by conducting pre-surgical screenings and risk assessments, arranging referrals and services, and providing education and emotional support throughout the surgical journey.
  • Acts as a patient advocate and navigator, collaborating with the interdisciplinary care team to develop individualized care plans, facilitate smooth transitions across all phases of care, support insurance and benefit utilization, and ensure effective postoperative follow-up and discharge planning.
  • Responsible for overall coordination and delivery of the Total Joint Education Class provided for patients preparing for total joint surgery.
  • Supports the ongoing success of the total joint program by tracking and reporting key performance metrics, identifying opportunities for quality improvement.
  • Collaborating with interdisciplinary teams to ensure consistent, high-quality care across the orthopedic service line.
  • Actively contributes to program development and organizational initiatives through participation in committee meetings and hospital-wide collaborations.
