Registered Nurse (RN) – Multi-Territory Home Health Clinician
About The Position
Home Health Care, Inc. is seeking a full-time Registered Nurse (RN) to work as a Floating Visit Nurse across the Greater Twin Cities Metro Area, covering a 21-county service area. This unique role offers the opportunity to experience the full scope of home health nursing by working with diverse patient populations, supporting multiple clinical teams, and caring for patients throughout the metro area. It is designed for nurses who are energized by learning, enjoy new environments, and want a role where every day feels different. The position provides consistent patient volume, exposure to a wider variety of diagnoses and care plans than traditional home health roles, and opportunities for continuous learning and professional development. The agency has been serving Minnesota for over 30 years and is Medicare-certified.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Reliable transportation with the flexibility to travel throughout the Twin Cities Metro
Nice To Haves
- Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
- Strong assessment and critical thinking skills
- Comfortable adapting to different clinical environments
- Excellent communication and time management abilities
Responsibilities
- Provide skilled home health nursing visits throughout the Twin Cities service area
- Complete comprehensive patient assessments
- Perform wound care, medication management, injections, and other skilled nursing procedures
- Educate patients and caregivers on treatment plans and disease management
- Coordinate care with physicians and interdisciplinary team members
- Provide coverage across territories to support patient needs and growing census
- Complete accurate documentation using Homecare Homebase (HCHB)
- Help ensure continuity of care while delivering exceptional patient experiences
