About The Position

As a member of the Health Support Center (HSC) team, you’ll support those that are in our facilities who are interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier ®. Acute care medical‑surgical unit 24‑bed unit with a 1:6 nurse-to-patient ratio 1:10 CA-to-nurse ratio Diverse patient population including medical, surgical, testing, and observation patients Common comorbidities: diabetes, hypertension, stroke, renal, and psychiatric conditions Strong team culture, improved nurse communication, and commitment to Sovah Health core values
Requirements
- An Associates Degree in Nursing (Bachelor’s degree preferred).
- A current VA (or Multi-state) RN license
- American Heart Association Basic Life Support

Responsibilities
- Performs plan of care interventions, including medication administration, specimen collection, and clinical treatments.
- Documents patient care and monitors patient condition, communicating with care team, physician, patient, and family.
- Assists patients with activities of daily living, including hygiene, elimination, nutrition, and ambulation.
- Coordinates patient care across disciplines, including transfers, discharges, referrals, and support needs.
- Provides patient and family education appropriate to age, culture, and circumstances.
- Advocates for the physical and emotional well-being of patients.
