About The Position

The Registered Nurse (RN) Manager oversees the daily management and coordination of the interdisciplinary team(s) involving Hospice in-home and/or facility-based care. Responsibilities include managing and planning of patient/resident care operations; ensuring all compliance standards are met. Supervises members of the IDG Team including Field RN's, LPN's, Therapists, PCA's, Social Workers, Pastoral Care, Nutrition Counselors, and contracted staff.

Requirements

  • Graduate of an accredited school of nursing with current RN licensure in the State of New York.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.
  • Two (2) years of clinical nursing experience required.

Nice To Haves

  • BSN preferred.
  • Certification in Hospice and Palliative Care Nursing preferred.
  • Hospice experience preferred.

Responsibilities

  • Ensures compliance with organization criteria and regulations as well as state and federal regulations as they apply to the Hospice programs; communicates updates/changes to staff.
  • Assists in the recruiting, hiring, developing and evaluating of key personnel; ensures clinically competent staffing levels are maintained.
  • Works with the Director of Community Based Services to establish goals and objectives including cost containment efforts.
  • Collaborates with the Director of Community Based Services, Chief Clinical Operations Officer and Director of Quality Improvement in the implementation of an agency wide Continuous Quality Improvement Program; assists in monitoring activities that are both patient and organizational focused to promote clinical quality.
  • Handles implementation of care to meet the physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient/family's well-being, comfort, and dignity throughout the dying process while maintaining standards of care consistent with the Hospice Philosophy which can be verified through outcome measures.
  • Must be able to competently oversee and perform all functions of the office and field RN/LPN duties, inclusive of all platforms and documentation requirements.
  • Prepares staffing schedules as appropriate and anticipates staffing profile needs; plans and participates in the orientation and staff development needs to assure that program personnel are current in clinical information and practices.
  • Maintains an appropriate schedule of supervisory field visits and assures that staff adheres to yearly Department of Health mandatory requirements.
  • Facilitates patient/family needs or problems and makes recommendations for changes in the POC in collaboration with the IDT, attending MD. Director of Community Based Services, and Patient Advocate.
  • Assures the accuracy and policy compliance of all initial, renewal and modified orders and reviews all PPOC's for procedural compliance.
  • Assures timely completion of all hospice recertification's and assists the IDG Team in documenting patient's status and needs to appropriately support the Hospice programs and level of care.
  • Completes fiscal responsibilities (i.e. pharmacy and outpatient invoice review) timely and assures that services and interventions (i.e. medications, advanced therapies) are included in the PPOC.
  • Initiates, completes and assures clinical oversight of advanced therapies per the Clinical Case Management Review process.
  • Assures that patient's insurance authorizations are monitored and obtained timely and any insurance changes are reviewed with the Director or Chief Clinical Officer.
  • Adheres to HIPAA regulations and policies.
  • Assures the security of patient electronic/written records.
  • Participate in the on-call manager rotation, responding to clinical after-hours needs with appropriate interventions and staff guidance to ensure 24/7 oversight of Hospice operations.
