Registered Nurse (RN)/Licensed Practical Nurse (LPN)
About The Position
Community Health Center of Central Wyoming (CHCCW) is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Casper clinic team. This position provides high-quality nursing care to patients in an outpatient community health center setting. The RN/LPN works closely with providers and other members of the care team to ensure patients receive safe, efficient, compassionate, and coordinated care. The ideal candidate will be patient-focused, dependable, organized, and comfortable working in a fast-paced clinical environment.
Requirements
- Current Wyoming RN license/LPN license or ability to obtain Wyoming licensure.
- Graduate of an accredited nursing program.
- Strong communication, organization, and critical-thinking skills.
- Ability to work effectively as part of a multidisciplinary healthcare team.
Nice To Haves
- Previous outpatient, family practice, primary care, or community health experience preferred.
- Experience working with diverse patient populations preferred.
- Experience with electronic medical records preferred.
Responsibilities
- Provide direct nursing care to patients in accordance with established standards, policies, and procedures.
- Perform patient assessments, obtain vital signs, and document patient information accurately in the electronic medical record.
- Assist providers with examinations, procedures, treatments, and other clinical needs.
- Administer medications and immunizations as permitted by licensure and organizational policy.
- Perform treatments, wound care, injections, and other nursing procedures within scope of practice.
- Provide patient education regarding medications, treatment plans, preventive care, and health management.
- Respond appropriately to patient questions, concerns, and changes in condition.
- Assist with telephone triage and coordinate appropriate follow-up care.
- Review and communicate laboratory and diagnostic results as directed.
- Maintain accurate, timely, and complete clinical documentation.
- Support care coordination and population health initiatives.
- Promote preventive care and patient engagement.
- Maintain patient confidentiality and comply with HIPAA and all applicable laws, regulations, and CHCCW policies.
- Participate in quality improvement and patient safety initiatives.
- Work collaboratively with providers, nurses, medical assistants, front office staff, and other members of the healthcare team.
- Perform other duties as assigned.
