Requirements

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts.
One year of nursing experience (home health or psych preferred).
A drive to provide compassionate, patient-centered care.
Reliable transportation for patient visits.

Responsibilities

Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
Create and Modify Care Plans: Actively shape and adjust patient treatment plans based on their needs and changing conditions.
Teach and Support: Provide guidance to patients, such as medication guidance for psychiatric patients or wound care for post-op patients, to help them stay independent at home.
Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive the right care at the right time.
Document and Reflect: Submit visit notes from home to ensure accurate record-keeping for compliance and continuity of care.
