About The Position

The Ambulatory and Outpatient Registered Nurse (RN) Care Coordinator, a member of Advocate Health Nursing Professional Governance community, provides high- quality, patient-centered care through evidence-based practice, clinical expertise, and compassionate care delivery in an ambulatory care setting. The RN collaborates with interdisciplinary teams, advocates for patient needs, and upholds professional practice standards to promote optimal health outcomes.

As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
Supporting the practice - covering the in basket and working with patient calls.
Coordinating pre and post procedure education and preparation
Post procedure assessments for pain.

Requirements

• Completion of an accredited or approved Bachelor’s Degree program in nursing
• Registered Nurse license issued by the state in which the teammate practices
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
• Typically requires 3 years of experience in clinical nursing that includes experience in nursing assessment, care plan development and in educating patients
• Working knowledge of the components of quality and patient care needs specifically related to the area in which care will be performed.
• Maintains clinical expertise to effectively manage physical, emotional, psychosocial and spiritual needs of patients throughout the care continuum.
• Excellent analytical and interpersonal communication skills necessary to negotiate with families, patient, physicians and third party payers.
• Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
• Clear oral communications and hearing acuity required for receiving instructions and converse on standard telephone.
• Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone.
