About The Position

Atrium Health Transition Clinic is a short term, ambulatory post discharge clinic providing multidisciplinary care to address barriers, improve access, and reduce readmissions. We are seeking a dedicated and experienced Registered Nurse (RN) to join our team, full-time. Ideal nursing candidates demonstrate a strong commitment to service excellence, nursing excellence, and adaptability. This is a fully remote, virtual role. The RN will be responsible for receiving and reviewing referrals following recent patient discharges, determining appropriate next steps, and scheduling patients with a provider or connecting them to the appropriate care team as needed. As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices

Nice To Haves

  • 3 years of nursing experience preferred
  • BSN preferred
  • Prior nurse navigation experience preferred
  • Ambulatory, triage, or care coordination/navigation experience preferred
