About The Position

As a Per Diem Hospice Registered Nurse (RN) for Intake/Admissions at The Community Hospice, you will be a vital part of our Central Intake & Admissions Team. Your role is crucial in ensuring patients and families receive compassionate and seamless care during a significant life transition. You will often be the first point of contact, setting a tone of comfort, clarity, and reassurance while guiding individuals through the process of beginning hospice care. This position offers a dynamic schedule, combining work in our central intake office in Albany with field visits to patients in their homes, nursing homes, and assisted living communities across the region.

Requirements

Active New York State RN license
ASN required
Valid driver’s license and reliable transportation
Strong communication skills—you connect easily with patients, families, and clinical teams
Clear, concise documentation abilities
Comfort with computers and electronic health records
Excellent organization and time management skills

Nice To Haves

BSN preferred
Home care experience (1+ year) strongly preferred
Hospice or end-of-life care experience is a plus

Responsibilities

Serve as a key liaison between patients, families, providers, and our interdisciplinary team.
Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams.
Meet patients where they are—at home or in care facilities—to complete initial assessments.
Initiate care plans, including ordering medications, medical equipment, and supportive services.
Partner with social workers, chaplains, aides, therapists, and volunteers to provide holistic care.
Ensure a smooth, supportive handoff to the Hospice Case Manager.
