About The Position

Atrium Health Infectious Disease Cabarrus is looking for an Ambulatory RN to support Triage, do Care Coordination for our at home infusion patients and support our patients in the clinic. This position is located at AH Cabarrus in the Medical Arts Building. This RN will work closely with all of our providers and other clinical support team. We are seeking a compassionate, skilled, and patient-focused Registered Nurse (RN) to join our Infectious Disease team. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and the ability to effectively manage patient care across multiple settings, including telephone triage, care coordination, and outpatient clinical support.

Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Current BLS certification

Nice To Haves

  • Minimum of 3 years of nursing experience
  • BSN
  • Ambulatory care, med-surg, home health, infectious disease, or care coordination experience
  • Strong assessment, triage, and patient education skills
  • Experience managing chronic and complex patient populations

Responsibilities

  • Provide and coordinate comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
  • Utilize evidence-based practice, clinical decision making, compassion, and communication skills while leading efforts to create the safest patient environment and the best patient experience across the continuum.
  • Support Triage.
  • Provide Care Coordination for at-home infusion patients.
  • Support patients in the clinic.
