This role involves performing the Nursing Process (assessment; diagnosis; identification of outcomes; planning; implementation and evaluation) in the performance of clinical care using evidence base practice, analytical/critical thinking, and ensuring individualized care. The nurse will coordinate care through effective partnerships, recognizing that caring is central to achieving optimal patient care outcomes. Relationship-based, patient-centered care consistent with population-specific characteristics will be provided, adapting service delivery to reflect an understanding of cultural diversity. The nurse will partner with the patient and significant others, using appropriate methods for setting and purpose (e.g., bedside shift report; interdisciplinary rounds; just-in-time and planned patient teaching; keeping the patient and significant others updated and making the patient’s goals the focus of the plan of care. Practice will be based on current clinical practice standards. Teamwork and collaboration are essential, coordinating the delivery and documentation of safe quality patient care that promotes the professional care delivery model. This includes diverse and inclusive interdisciplinary communication methods (interdisciplinary rounds, case review, etc.), timely documentation, and promoting a respectful, inclusive clinical environment. The role requires practicing in a fiscally responsible manner and promoting the recruitment and retention of all healthcare team members. Participation and support of performance improvement, research, and research utilization to promote safe, quality patient care are expected, including initiating or leading such activities and supporting shared governance. Professional development includes completing all initial and ongoing professional competency assessments, required mandatory education, and population-specific education. The nurse will serve as a preceptor and/or mentor for other professional nurses and staff or students, modeling the professional practice of nursing and creating a healthy work environment. Evidence-based practice and research promotion are key, participating in activities such as value analysis. Upholding all health care system/organizational policies and procedures, clinical competencies, and legal, regulatory, and accreditation requirements is mandatory. Participation in data collection and posing relevant clinical questions to advance evidence-based practice is also required. Proactive planning for patients across the care continuum is expected. Participation in the development of protocols and procedures, in collaboration with care managers and other healthcare team members, to achieve best practice outcomes is also a responsibility. Support for efficient and effective use of human and material resources is required.
