The primary functions of this role are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency, and assume the responsibility for coordination of care. This position involves performing initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients' physical, functional, psychosocial, and environmental needs. It also includes applying specific criteria for admission and re-certification to hospice care, implementing and developing the plan of care, and consulting with and educating the patient and family regarding disease process, self-care techniques, end-of-life care, and nutrition. The role requires prioritizing needs with the IDG, providing training to other staff as needed, determining the scope and frequency of services, assessing caregiver ability, initiating preventive and rehabilitative nursing procedures, and applying infection control concepts. Additionally, the RN will provide clinical directions to the Hospice Aide and LPN/LVN, prepare clinical and progress notes, ensure continuity of care, inform the physician of patient changes, monitor cases for third-party payer compliance, and demonstrate commitment to professional growth and competency. The position also involves promoting agency philosophy and policies, and providing effective communication.
