As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most.

Requirements

Active RN license in the state of employment
Graduate of an accredited school of Nursing.
Compassion for patients and families navigating end-of-life care
Strong clinical judgment and the ability to work independently in the field
Excellent communication skills across care teams and families

Nice To Haves

One (1) + year Hospice, home health or related organization experience preferred.

Responsibilities

Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
Building and adjusting personalized care plans in collaboration with physicians and care teams
Administering medications and treatments per physician orders
Keeping clinical documentation accurate, timely, and meaningful
Serving as the communication bridge between families, physicians, and community health partners
Educating patients and caregivers so they feel confident and supported
Participating in on-call rotation per team policy
