About The Position

This is a part-time Registered Nurse (RN) position for 16 hours per week, specifically on Saturday and Sunday, within the hospice division of Home Health & Hospice Care (HHHC). The role involves providing hospice care to patients in their own homes and in facilities/nursing homes in Southern New Hampshire, including the greater Merrimack, Nashua, and Manchester communities. The position is supported by a highly skilled Manager and Team Lead for hospice/end-of-life care. Requirements

  • Graduate of an accredited school of nursing.
  • Associate degree in nursing.
  • Registered as RN with New Hampshire’s Board of Nursing.
  • A minimum of one year recent acute care experience.
  • Ability to exercise initiative and independent judgment.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.
  • CPR Certification.

Nice To Haves

  • EPIC experience preferred, willing to train!
  • Bachelor degree in nursing.
  • Community Health/Education experience.
  • Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
  • EMR software proficiency.

Responsibilities

  • Completes an initial home health assessment of patient and family to determine hospice care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Utilizes the nursing process in planning, providing and evaluating skilled needs.
  • Completes a comprehensive patient assessment.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Includes the patient and family on the planning and revising process.
