As an ECT (Extended Care Team) Nurse at Transitions Hospice, you're the reinforcement — the skilled, composed clinical presence that steps in to ensure no patient need goes unmet. This isn't a role for someone who needs a rigid routine. What You'll Do As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include: Delivering professional nursing care across the full nursing process within your scope of practice Conducting thorough patient and family assessments and contributing to individualized care plans Completing all visit types as needed to meet patient and family needs Administering medications and treatments per physician orders Identifying nursing diagnoses from health assessment data and responding accordingly Recording accurate, timely clinical notes including pain and symptom management outcomes Communicating changes in patient condition to physicians and obtaining orders as required Coordinating with community health partners to support seamless care transitions Ensuring equipment and necessary services are arranged and available for patients Maintaining and growing your clinical skills through continuing education and in-service participation
