About The Position

Show Up When It Matters Most In hospice care, there are moments when a patient or family needs more than their regular care team can provide in that instant. That's where you come in. As an ECT (Extended Care Team) Nurse at Transitions Hospice, you're the reinforcement — the skilled, composed clinical presence that steps in to ensure no patient need goes unmet.
