About The Position

This role is for a compassionate Registered Nurse (RN) Team Nurse who will support the hospice clinical team by providing skilled nursing visits, assisting with patient coverage, completing scheduled assessments, and ensuring every patient receives exceptional care, especially when their primary nurse is unavailable. It's an opportunity for an experienced nurse who values independence in the field while being backed by a collaborative interdisciplinary team. The Hospice Team Nurse will partner with RN Care Managers and other clinical professionals to deliver high-quality hospice care throughout Columbus and the surrounding service area.

Requirements

  • Current Ohio Registered Nurse (RN) license in good standing
  • Graduate of an accredited nursing program
  • Associate Degree in Nursing (ADN/ASN) or higher
  • Current CPR certification
  • Valid driver's license, dependable transportation, and state-required auto insurance
  • Comfortable traveling throughout the service area
  • Experience using electronic medical records

Nice To Haves

  • Bachelor's Degree in Nursing (BSN)
  • Two or more years of nursing experience in acute care, home health, hospice, or a related clinical setting
  • Hospice and Palliative Nurse (CHPN) certification, or interest in obtaining certification

Responsibilities

  • Completing routine and scheduled nursing visits, including 14-day assessments and other required clinical visits
  • Providing coverage for RN Care Managers during PTO, leave, or scheduling needs
  • Performing comprehensive patient assessments and identifying changes in condition
  • Developing, updating, and implementing individualized plans of care
  • Collaborating with physicians, social workers, chaplains, aides, and the interdisciplinary team to coordinate patient care
  • Educating patients, families, and caregivers on disease progression, symptom management, medications, and the hospice plan of care
  • Communicating changes in patient condition and obtaining physician orders as appropriate
  • Completing accurate, timely electronic documentation
  • Supporting quality improvement initiatives and maintaining compliance with hospice regulations
  • Serving as a mentor or preceptor for new clinical staff when assigned
