About Duke HomeCare & Hospice Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home for the Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC. Requirements

  • Associate Degree or Diploma in Nursing required; BSN encouraged after two years.
  • Current RN license in North Carolina or compact state.
  • CPR certification (American Heart Association).
  • Minimum two years of med-surgical, critical care, or home health/hospice experience required.
  • Prior hospice experience required.
  • Valid driver’s license and current automobile insurance.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work independently, travel for patient care, and support on-call responsibilities.

Nice To Haves

  • BSN encouraged after two years.

Responsibilities

  • Provide professional hospice nursing care in home or alternate settings using the nursing process and regulatory standards.
  • Assess patient conditions, manage symptoms, and communicate significant changes to physicians and care teams.
  • Educate patients and families on disease progression, medications, and end-of-life care planning.
  • Collaborate with interdisciplinary teams to coordinate care, support discharge planning, and ensure quality outcomes.
  • Maintain accurate documentation, ensure compliance, and support performance improvement initiatives.
