About The Position

As an RN Case Manager, you will play a vital role in coordinating care, supporting patients and families, and helping individuals thrive in the comfort of their homes. You will oversee and coordinate patient care while working closely with an interdisciplinary team. This role involves developing, implementing, and updating individualized plans of care, performing comprehensive patient assessments, and providing skilled nursing care. You will also educate and counsel patients, families, and caregivers, coordinate care with physicians and other healthcare professionals, and manage documentation, with a majority completed in the patient’s home. Additionally, you will lead and participate in case conferences, identify discharge planning needs, communicate patient status updates, supervise LPNs and Home Health Aides, and ensure necessary equipment and services are arranged for patients.

Requirements

  • Current Registered Nurse (RN) license in the State of Wisconsin
  • Graduate of an accredited nursing program
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and organizational abilities
  • Ability to work independently and manage a caseload effectively
  • Valid driver’s license with a reliable, insured vehicle
  • Physical ability to perform patient care tasks (lifting, standing, mobility support, etc.)

Nice To Haves

  • Home health or public health experience preferred

Responsibilities

  • Developing, implementing, and updating individualized plans of care
