The best career is right here. St. Elizabeth Healthcare is hiring a Hospice Care Coordinator for the Hospice unit located at the Edgewood, Kentucky location. Shift & Hours: 1st shift; PRN (less than 20 hours weekly). Establish and maintain agency contacts with area health facilities and community organizations. Facilitate client transfer between the facilities and the hospice home care and inpatient unit to ensure continuity of care. Provide education to the community on the Hospice Philosophy of Care. Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background. Requirements

  • Graduate of an accredited school of nursing; licensed to practice in the state where work is being performed.
  • If eligible, must have a Compact (multi-state) License in their primary state of residence.
  • Meets contact hour requirements for licensure, including all state-required courses.
  • Valid Driver’s license.
  • Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.
  • 2-3 years of RN experience in a hospital or care facility setting.
