About The Position

Commonheart is seeking a Registered Nurse for both Home Health and Hospice roles. This position offers the opportunity to provide meaningful, one-on-one care and make a real difference in patients' lives. The role involves providing education and counseling to patients and their families, participating in in-service training and continuing education, and completing all documentation in a timely manner, including ongoing observation and assessment of patient/family needs.

Requirements

• Must be currently licensed as a Registered Nurse in the State of Wisconsin.
• Commitment to quality outcomes and services for all individuals.
• Ability to relate well to all individuals.
• Ability to maintain and protect the confidentiality of information.
• Ability to exercise independent judgment and make sound decisions.
• Ability to adapt to change.
• Must be sensitive to the need of older adults.

Nice To Haves

• At least 2 years of experience in Medical Surgical, Home Health, Hospice, or Long-Term care nursing preferred.
• Certified in Gerontology, Medical Coding, Wound Care (WCC), Hospice and Palliative Nurse preferred.

Responsibilities

• Provide education and counseling to the patient and, as needed, to the patient’s family, consistent with the plan of care.
• Actively participate in Home Health & Hospice sponsored in-service training, and continuing education opportunities.
• Complete all documentation in time frame determined by policy, including ongoing observation and assessment of patient / family needs.
