About The Position

This role focuses on providing after-hours patient care for hospice patients in Bloomington, IN, and surrounding areas. The Registered Nurse (RN) will make a difference in patients' lives by enabling them to spend quality time with loved ones in their preferred environment. The position involves responding to after-hours patient needs, providing one-on-one patient care, and working collaboratively within an interdisciplinary team dedicated to compassionate, quality care and positive patient outcomes. The role also includes interacting with patients' families and supporting their recovery journey.

Requirements

  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Designated workspace at home to assist with chart reviews if there are minimal to no after-hours needs.
  • Capable of all physical demands.
  • Employee must be free of physical/medical conditions that would limit or restrict their ability to perform the job functions.

  • Home care experience preferred.

Responsibilities

  • Respond to after-hours patient needs.
  • Provide one-on-one patient care.
  • Support a team of nurses and patients.
  • Provide skilled nursing care in a patient's home as directed by the attending physician.
  • Work autonomously and manage time efficiently.
  • Observe and monitor patient conditions.
  • Perform OASIS assessments to develop and adjust individualized care plans.
  • Administer medication as prescribed by the physician.
  • Prioritize visits for high-risk patients to decrease re-hospitalizations.
  • Oversee and supervise total care of patients provided by nurse aides and LPNs.
  • Manage multi-disciplinary care as applicable, promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Educate the patient and family on the disease process using teach-back methods.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal care.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
  • Complete chart reviews, if needed based upon after-hours needs, to support patient care and regulatory compliance.
  • Help with chart review or patient phone calls from home if there are minimal to no after-hours needs during the shift.
