About The Position

The RN After Hours utilizes the nursing process and evidence-based practice to provide patient-driven, holistic care with the collaboration of the Interdisciplinary Team, Medical Director, Primary Care Physician or Attending Physician and Expanded Team (family/caregiver and internal and community-based services involved in providing care to the patient). Develops care protocols that support care for patients receiving hospice services throughout the continuum to assure that care is timely, appropriate, of high quality, and cost effective. Uses health education and technology to promote therapeutic relationships and share decision making between the patient and the interdisciplinary team to support patient self-management, prevention and comfort. Demonstrates leadership in the delivery of care through collaborative strategies with others. The RN After Hours will provide skilled nursing care in a patient's home as directed by the medical director and/or attending physician. The RN After Hours will manage and facilitate resource applications to meet patient needs. If there are minimal to no after-hours needs during the shift, the RN On-Call will support chart review and patient phone calls from their home. They are responsible for all practices and duties within the scope of practice outlined by the state. The RN After Hours is responsible for any and all practices and duties within the state specific scope of practice.

Requirements

  • Registered Nurse with current license in the state of employment.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
  • Ability to remain calm, have patience and be accommodating.
  • Compassionate and caring while working with patients.
  • Knowledgeable on nursing best practices.
  • Ability to make appropriate nursing judgments.

  • Detail-oriented and observant.
  • Ability to work autonomously and have efficient time management.
  • Ability to follow directions and work as a team member.
  • Valid driver’s license and auto insurance with your name as a listed driver.
  • Designated workspace at home to assist with chart reviews.

Nice To Haves

  • 2 years of hospice experience preferred.
  • Previous experience with clinical management, hospice case management, or other relevant clinical experience.
