About The Position

The RN-Home Infusion Clinical Liaison serves as the clinical liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to support timely initiation of home infusion therapy. Provides clinical screening, coordination of services, and patient and caregiver education to ensure safe and effective infusion care in the home setting. Supports business development by building strong relationships with referral partners and promoting appropriate use of home infusion services. Collaborates closely with intake, pharmacy, nursing, and operational teams to ensure patients receive the right therapy at the right time.

Requirements

  • Associates degree in Nursing.
  • One (1) year of nursing experience.
  • Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
  • A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.
  • Driving is required for this position using the employee’s personally owned vehicle.
  • Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
  • Employee must pass driver training.
  • Experience with patient education
  • Knowledge of Regulatory and Payor standards and requirements

Responsibilities

  • Serves as the primary clinical liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to facilitate timely initiation of home infusion therapy.
  • Performs clinical screening and assessment of referred patients to evaluate appropriateness of home infusion therapy based on diagnosis, prescribed treatment, patient readiness, and home environment considerations.
  • Collaborates with intake coordinators, pharmacists, nursing staff, and providers to coordinate therapy initiation, nursing services, medication delivery, and start of care timing.
  • Educates patients and caregivers on prescribed infusion therapies, vascular access devices, equipment use, and safety considerations to support successful therapy in the home setting.
  • Communicates clinical information and recommendations to referring providers, case managers, and discharge planners to ensure continuity of care and appropriate transitions from inpatient or outpatient settings.
  • Identifies and addresses clinical barriers to home infusion therapy initiation, escalating complex cases or concerns to leadership or medical providers as appropriate.
  • Supports business development efforts by building and maintaining relationships with hospitals, clinics, physicians, and referral partners, serving as a clinical resource to promote home infusion services.
  • Participates in referral source education and outreach activities to increase awareness of home infusion capabilities, clinical appropriateness, and service offerings.
  • Ensures accurate, timely documentation of clinical assessments, patient education, and coordination activities in accordance with regulatory, accreditation, and payer requirements.
  • Participates in quality improvement and performance initiatives related to home infusion clinical services, contributing clinical insight to improve patient outcomes and operational efficiency.
  • Performs other duties as assigned.
