About The Position

Matrix Home Care is seeking a dedicated and experienced Registered Nurse (RN) to provide home health services in the Wellington and Royal Palm Beach areas. This role includes Start of Care (SOC) visits, routine skilled nursing visits, and ongoing patient care management in a home setting.

Requirements

  • Active Florida Registered Nurse (RN) license in good standing
  • Minimum 1 year of home health experience required
  • Strong experience with Start of Care (SOC) and OASIS documentation
  • Experience with KanTime EMR system preferred
  • Excellent clinical assessment and critical thinking skills
  • Ability to work independently and manage a flexible schedule
  • Reliable transportation and valid driver’s license

Responsibilities

  • Provide home-based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility
  • Perform Start of Care (SOC) visits, comprehensive assessments, and develop individualized care plans
  • Examine patients, complete clinical assessments, document progress, and report changes to physicians
  • Complete accurate and timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes
  • Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives
  • Deliver skilled nursing services and provide case management for assigned patients
  • Educate patients on plan of care, medications, therapies, diet, and exercise
  • Coach and support family members on patient care, medication management, and home safety
  • Coordinate care with physicians, discharge planners, and interdisciplinary teams
  • Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care
  • Monitor patient progress, outcomes, and changing needs, adjusting care plans accordingly
  • Ensure care goals are met and coordinate appropriate discharge from services
  • Serve as a liaison with community health resources, including DME providers, oxygen services, and infusion therapy providers
  • Maintain ongoing communication with patients, families, physicians, clinical managers, and office staff
  • Participate in interdisciplinary team meetings and staff conferences
  • Engage in continuous education and professional development opportunities
  • Develop and follow work methods and procedures that support high-quality patient care
