About The Position
Preferred Care Home Health is seeking a Full-Time Registered Nurse (RN) to provide skilled home health care to patients in assisted living, independent living, and other residential care communities in the Naples, FL area. Founded in 2004 by a Registered Nurse, Preferred Care has grown into one of Florida's leading home health agencies. Requirements
- Graduate of an accredited nursing program
- Active, current Florida RN license
- Current CPR/BLS certification
- Minimum 5 years of RN experience
- Strong assessment, communication, and organizational skills
- Comfortable working independently while collaborating with a team
Nice To Haves
- Home health and OASIS documentation experience preferred
Responsibilities
- Perform comprehensive nursing assessments and admissions for home health patients
- Develop and manage individualized plans of care alongside physicians and the care team
- Deliver skilled nursing visits per physician orders and agency policy
- Monitor patients closely and report changes in condition promptly
- Educate patients, families, and caregivers on disease management, medications, and safety
- Complete OASIS assessments and documentation accurately and on time
- Coordinate with therapists, social workers, physicians, and facility staff
- Maintain compliance with Medicare, CMS, and ACHC standards
- Participate in case conferences and interdisciplinary team meetings
