Requirements

Active Registered Nurse (RN) license in Massachusetts.
One year of nursing experience.
A drive to provide compassionate, patient-centered care.
Reliable transportation for patient visits.

Nice To Haves

Home health or psych nursing experience preferred.

Responsibilities

Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
Create and Modify Care Plans: Actively shape patients' treatment plans based on their needs and adjust care as conditions change.
Teach and Support: Guide patients (e.g., psychiatric patients needing medication guidance, post-op patients learning wound care) to help them stay independent at home.
Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive the right care at the right time.
Document and Reflect: Submit visit notes from home for accurate record-keeping and continuity of care.
