Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- One year of nursing experience (home health or psych preferred).
- A drive to provide compassionate, patient-centered care.
- Reliable transportation for patient visits.

Responsibilities
- Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
- Create and Modify Care Plans: Shape patient treatment plans based on their needs and adjust care as conditions change.
- Teach and Support: Provide medication guidance to psychiatric patients or wound care education to post-op patients to help them stay independent at home.
- Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive the right care at the right time.
- Document and Reflect: Submit visit notes from home for accurate record-keeping and continuity of care.
