Registered Nurse, RN - Home Health
About The Position
BAYADA Home Health Care is seeking experienced home health Registered Nurses (RNs) to join their home health visit teams in North East Massachusetts. They are offering incentives for some territories and are currently hiring for multiple full-time roles, including Start of Care RN, Preceptor, Case Manager, and Clinical Associate. These positions require experience with OASIS. The RN will be an integral member of a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients, enabling them to remain at home. BAYADA believes clients and their families deserve the highest quality home health care delivered with compassion, excellence, and reliability. A $5,000 incentive is offered for a full-time commitment in Somerville and surrounding territories.
Requirements
- A current nursing license in good standing in the state of Massachusetts
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)
- Experience with OASIS

Responsibilities
- Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
- Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
- Triage and manage urgent clinical situations independently in the home setting
- Perform medication reconciliation, teaching, and safety monitoring
- Perform comprehensive wound assessment, care, and documentation
