Requirements

Active Registered Nurse (RN) license in Massachusetts.
One year of nursing experience (home health or psych preferred).
A drive to provide compassionate, patient-centered care.
Reliable transportation for patient visits.

Responsibilities

Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
Create and Modify Care Plans: Shape patients' treatment plans based on their needs and adjust care as conditions change.
Teach and Support: Provide guidance on medication, wound care, and other aspects to help patients stay independent at home.
Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure timely and appropriate care.
Document and Reflect: Submit visit notes accurately from home for compliance and continuity of care.
