This position is for a Home Health Registered Nurse (RN) within LV Home Care and Hospice, working full-time days with a $10,000 sign-on bonus. Requirements

  • Specialized Diploma Nursing
  • 1 year Recent direct care experience in a healthcare setting.
  • Ability to build comprehensive reports and effectively convey information to others.
  • Ability to deal with crisis situations and make independent judgments.
  • Knowledge of patient education techniques and principles.
  • Knowledge of home care Conditions of Participation.
  • Skill in providing patient care, ensuring safety, and quality of nursing care.
  • Basic computer skills and ability to perform duties with accuracy and attention to detail.
  • Demonstrates flexibility and adaptability in varying situations to meet patient care needs.
  • Demonstrates initiative and ability to function independently without direct supervision and as a member of the care team.

Responsibilities

  • Performs a comprehensive subjective and objective assessment using standardized assessment tools to gain pertinent details about the patient's overall health, well-being, abilities, and living environment; performs on-going assessment of physical, psychosocial, and environmental parameters.
  • Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach.
  • Reviews, evaluates, and as appropriate, revises the established plan of care for each patient.
  • Collaborates with the interdisciplinary team and other members of the patient's care team in assessing and supporting psychosocial needs of the patient/family members/caregiver.
  • Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning.
  • Promotes shared governance, facility decision making activities, and developing and nurturing research to positively affect clinical outcomes and promotion.
  • Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible.
  • Directs and guides ancillary personnel and maintains standards of professional nursing.
