About The Position

Big Bend Hospice (BBH), a proud subsidiary of Seven Oaks Health, has delivered compassionate, high-quality care for more than 40 years across 12 counties in the Big Bend area. As we prepare to launch our new Home Health Team in June 2026, we are excited to welcome an experienced Registered Nurse to help shape this growing service line. Guided by our core values of integrity, respect, and excellence, we are seeking a dedicated and skilled professional who is passionate about supporting patients and families in their homes and communities. The ideal candidate is a licensed Registered Nurse (RN) who is committed to delivering high-quality, patient-centered care in the home setting. This individual will excel in clinical assessment, care planning, and coordination while working both independently and collaboratively with an interdisciplinary team.

Requirements

  • Associate or Bachelor’s Degree in Nursing required.
  • Active Florida Registered Nurse (RN) license required.
  • Current BLS or CPR certification required.
  • Minimum of one (1) year of clinical nursing experience required.
  • Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

  • Provide comprehensive nursing assessments and develop individualized Plans of Care in accordance with physician orders.
  • Deliver skilled nursing care and interventions based on patient needs and clinical condition.
  • Monitor patient progress and adjust care plans as appropriate.
  • Administer medications and treatments in accordance with physician orders and organizational policies.
  • Provide patient and caregiver education on disease management, medications, safety, and self-care.
  • Coordinate care with physicians, therapists, and interdisciplinary team members.
  • Participate in discharge planning, including patient/family education and follow-up care recommendations.
  • Maintain accurate, timely documentation and complete visit notes within required timeframes.
  • Manage assigned caseload, assist with scheduling, and meet productivity expectations.
  • Ensure compliance with infection control standards, patient safety protocols, and regulatory requirements.
  • Demonstrate professionalism, compassion, and cultural competence in all patient interactions.
  • Perform other duties as assigned.
