Requirements

Active Registered Nurse (RN) license in Massachusetts.
One year of nursing experience (home health or psych preferred).

Reliable transportation for patient visits.

Responsibilities

Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
Create and Modify Care Plans: Shape patient treatment plans based on their needs and adjust care as conditions change.
Teach and Support: Provide guidance to patients, such as medication management for psychiatric patients or wound care for post-op patients, to help them stay independent at home.
Communicate and Coordinate: Inform providers, families, and care teams about changes to ensure patients receive timely and appropriate care.
Document and Reflect: Submit visit notes accurately from home for compliance and continuity of care.
