About The Position

Lux Home Health is a premier provider of home health care services. We are currently seeking a Home Health Registered Nurse (RN) with at least two years of nursing experience to join our dedicated team.

Requirements

Registered Nurse (RN) License in the state of Kansas.
At least 2 years of nursing experience, preferably in home health.
Current CPR certification.
Strong clinical skills and experience in providing skilled nursing care in a home setting.
Ability to work independently, manage a patient caseload, and prioritize tasks effectively.
Valid KS driver’s license, reliable transportation, and the ability to travel within the service area.
Excellent communication, interpersonal, and organizational skills.
A compassionate, patient-centered approach to care.

Responsibilities

Performing thorough initial assessments and developing individualized care plans that address each patient’s unique needs and health goals.
Providing wound care and other treatments as prescribed by physicians.
Collaborating with physicians, therapists, and family members to ensure a coordinated approach to patient care.
Maintaining accurate, up-to-date documentation of all patient care activities, while adhering to Lux Home Health’s internal policies and regulatory standards.
Guiding patients and their families on managing medical conditions, taking medications, and navigating their overall health.
Responding promptly to any medical emergencies or patient concerns, delivering professional care and appropriate interventions when necessary.
