About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes throughout the Naples, FL area (Collier county). Empath Home Health delivers high-quality, outcomes-driven care trusted by patients, families, and healthcare professionals. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Requirements

• Graduate of an approved nursing program with an active Florida RN license
• Minimum of 1 year of direct patient care experience
• Willingness to participate in weekend and holiday on-call rotations

Nice To Haves

• BSN or MSN degree
• Prior home health RN experience
• Experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
