About The Position
Elite Home Health is a Medicare-certified home health agency serving communities across Florida. We provide compassionate, high-quality care that helps patients recover, maintain independence, and improve their quality of life. We are seeking a dedicated Registered Nurse (RN) to provide and coordinate skilled nursing care for patients in their homes. This role is for a full-time position covering the Leesburg & Surrounding Areas. We are looking for a compassionate, detail-oriented nurse who values clinical excellence, teamwork, and patient-centered care.
Requirements
- Graduate of an accredited School of Nursing (ASN or BSN)
- Current Florida RN license in good standing
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Ability to meet all required background and compliance requirements
Nice To Haves
- Minimum 1 year of nursing experience preferred
- Home health, hospice, or acute care experience preferred
- Strong clinical judgment and critical thinking
- Excellent assessment and communication skills
- Ability to work independently in a home health setting

Responsibilities
- Perform comprehensive and ongoing patient assessments
- Complete Start of Care (SOC), recertification, reassessment, and other required visits
- Complete OASIS assessments accurately and within required timeframes
- Develop, implement, and update individualized plans of care
- Provide skilled nursing services in accordance with physician orders and the plan of care
- Monitor patient condition and identify changes in health or functional status
- Communicate significant changes and patient needs to physicians and the care team
- Coordinate care with physicians, therapists, LPNs, CNAs, and other interdisciplinary team members
- Provide direction and supervision to LPNs and CNAs as applicable
- Complete accurate and timely clinical documentation in the EMR
- Ensure documentation supports medical necessity and regulatory requirements
- Participating in care coordination, team meetings, and required training
