About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) to join our home health team. In this role, you will deliver one-on-one patient care, ensuring the highest standards of health and well-being for our clients. Weekend differentials are offered, along with licensure reimbursement.

Requirements

  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Valid driver's license required.
  • Basic Life Support (BLS) required.

Nice To Haves

  • Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Bachelor's degree in nursing preferred.
  • Minimum of one year of professional nursing experience preferred.
  • Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.

Responsibilities

  • Utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients at home.
  • Documenting human responses to actual or potential health problems in the home health environment.
  • Collaborating with patients, families, and other inter-professional colleagues, such as physicians, to plan, implement, and evaluate care.
  • Demonstrating competency and practicing within the full scope of nursing expertise/knowledge and utilizing appropriate age and population specific standards.
  • Caring for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up.
  • Implementing the nursing process within Sanford, coordinating continuum of care, patient assessment, patient education, triage, and various other nursing interventions.
  • Functioning within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.
  • Adhering to the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association.
