Requirements

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience.

• Reliable transportation for patient visits.

• Home health or psych experience preferred.

Responsibilities

• Assess and Adapt: Evaluate patient's current condition, medical needs, and overall well-being during each visit.
• Create and Modify Care Plans: Shape patients' treatment plans based on their needs and adjust care as conditions change.
• Teach and Support: Guide patients (e.g., psychiatric patients needing medication guidance, post-op patients learning wound care) to help them stay independent at home.
• Communicate and Coordinate: Keep providers, families, and care teams informed of any changes to ensure patients receive the right care at the right time.
• Document and Reflect: Submit visit notes from home, ensuring accurate record-keeping for compliance and continuity of care.
